HHCSRC Inc. [Holistic Health Care Society & Research Center Inc.] CONTRACT OF DISCLOSURE, INFORMED CONSENT, DISCLAIMER, TERMS, CONDITIONS & REGISTRATION 1. I understand & agree that, HHCSRC Inc. merchandise is NOT PRESCRIPTION MEDICATION. The HHCSRC Merchandise have NOT been evaluated by FDA. Merchandise skin & hair care product of the HHCSRC Inc. contains dietary supplements, herbs, vitamins, minerals, amino acids, enzymes, tissues from organs or glands, etc. 2. I was told repeatedly at the time of making appointment, that I should discuss with my family about using these merchandise and take their consent. I am an adult, do not suffer from any mental conditions. I am mentally competent to make my own decision & am fit to make payment INDEPENDENTLY if I decide to buy Merchandise. I hereby intend to be legally bound, for myself, my heirs, executors, administrators, personal representatives, successors or assigns by this contract. 3. I understand & agree that the HHCSRC Inc. and its employees cannot answer personal and specific health questions. They do not give advice, make recommendations, suggestions, claims, or guarantee a possible time frame and what Merchandise one should take. These types of questions are best reserved for my health care provider. Our conversation and opinion is only about products of HHCSRC Inc, and for informational purposes only. 4. I hereby agree that I have been fully informed about all these details, as well as the pros and cons about Merchandise intend to purchase from the HHCSRC Inc. I know that these Merchandise are not intended to diagnose, treat, cure, or prevent any disease or medical condition, or to replace professional medical advice/treatment. HHCSRC Inc. does not provide emergency services. If you want a medical diagnosis or treatment, please contact the New York County Medical Society for referrals. I am told that, I should discuss my using this merchandise with my physician before I purchase merchandise for its effects and any drug interaction. I have been told not to stop my prescription medicines without the consent of my health care provider when I start using this merchandise. 5. I understand Dr K. Rao is just an employee of HHCSRC. Inc and does not own this corporation and does not have any shares in this corporation. My contract & payment is with HHCSRC Inc. Future disputes will be restricted to HHCSRC Inc and NOT personally with its employees. 6. I understand & agree that I am not here in any medical doctor’s or natural doctor’s office to seek any medical services likes physical examination /investigation/ medical advice/ prescription/ physiotherapy, etc. Dr. K. Rao is not a medical practitioner. [Dr. Rao has a Bachelor of Medicine and Bachelor of Surgery Degree from India which is the equivalent of an MD in the USA. He is a past member of the American Academy of Pediatrics. However, as he does not wish to practice prescription medicine, he has not applied for and completed the board certification and licensing in the USA. He is not a licensed medical doctor in the USA and does not advice, examine, diagnose, cure, and treat any disease or recommend Merchandise. Dr Rao only provides Merchandise for purchase. All communications and discussions about the Merchandise are strictly for information purposes only. 7. Our Merchandise come under the category of FOOD not of medicines. Hence, there is no need to take FDA approval for its sale. However they are prepared under strict regulatory guidelines. Also FDA approval needs lot of trials and money. No drug company would pursue FDA approval for use of Merchandise because they can’t be patented by one company and there would be no profit incentive. Same as in case of generic drugs. 8. I understand & agree that there is no assurance, fix time frame or guarantee of any kind regarding any benefit from these HHCSRC Inc. Merchandise. The HHCSRC Inc. only supply these Merchandise specifically requested by me. I understand that individual results vary with Merchandise & they may or may not be suitable to me. 9. My issues are very complex and chronic. I also understand that there may be problems due to side effects of prescription drugs. Due to this I may develop acute complications which may coincide with the use of these Merchandise. I & my family have thoroughly thought about all these pros and cons and I am purchasing this merchandise with the consent of my family. I have been told that this product may contain other ingredients unintentionally introduced in the raw material, or during harvesting, or during the manufacturing process. It may contain substances which could cause an allergic reaction. It is my decision alone to purchase these Merchandise. I agree that I will use these Merchandise & HHCSRC Inc., or its employees will not be held responsible if any kind of complications, allergies or side effect occurs. 10. I authorize that the my investigative reports about me, disease progress, and audio-visual recording be used for the betterment of people, in scientific studies, publications, advertisements, television shows, the internet, on websites, and in other media outlets. I have agreed on my own will to record my views about the effects of these Merchandise while I am taking them. I have also agreed that my friends/family members can express their views about the effects of this merchandise. The HHCSRC Inc. has the sole copyright of these recordings and reports and I have given them the authority to use them as and when required. I, my family will not take any kind of legal action against the HHCSRC Inc. and/or Dr. Rao, and any media agency for publishing and broadcasting these clippings. 11. RETURN/REFUND POLICY- I agree that these Merchandise are mixed for my overall benefit and as soon as the order is placed the mixing of merchandise commence. AND ONCE THEY ARE MIXED EXCLUSIVELY FOR ME THEY CANNOT BE EITHER SEPARATED OR USED FOR ANYBODY ELSE AND HENCE CANNOT BE REFUNDED OR RETURNED. Hence, all sales of merchandise are final and there is No Guarantees, No cancellation of orders, No returns, No refunds and No exchanges. THEREFORE WE ARE TOLD MANY TIMES THAT IF I & MY FAMILY ARE NOT SURE OF THE PURCHASE, OR FOR ANY REASON IF WE INTENT TO LITIGATE FOR RETURN, REFUND OR OTHER REASONS IN FUTURE, THEN WE SHOULD KEEP OUR MONEY WITH US AND NOT PLACE ORDER OR PURCHASE THESE MERCHANDISE IN THE FIRST PLACE. I know that I am totally responsible for my purchase and I & my family can’t ask for return/exchange/refund in future either on the phone or in person. I fully understand & agree that I can’t dispute this payment transaction anywhere, including the Credit Company, bank or court of law. I understand that the HHCSRC Inc. doesn’t accept any personal check. If I do make the payment through a check/credit card, then I fully understand that I will be responsible for any decline, bank charge, stop payments, check returned and be liable to repay the charges with dues, including attorney’s fees, legal charges and/or collection agency fees. 12. I understand & agree that I have to pay $100 initial fee for discussion, which is non-refundable. After that if I buy the Merchandise, I have to pay the cost of the Merchandise and shipping & handling separately. 13. Any legal disputes/litigation will be only between the buyer & the HHCSRC Inc. This any employees and volunteers personally. All disputes shall be resolved only within the Jurisdiction of the HHCSRC Inc.’s present location Queens county court, New York, USA, at the time of filing of litigation. I, my family & agents hereby agree, not to elect trial by jury of any issue fully, knowingly and voluntarily and is intended to encompass individually each instance and each issue as to which the right to a trial by jury would otherwise accrue. HHCSRC Inc and its representatives are hereby authorized to file a copy of this contract in any proceeding as conclusive evidence of this waiver. 14. If I feel the need of dividing my Merchandise in doses for my benefit, then upon my request the Merchandise will be re-packed in paper packets/Ziploc bags. I know that the Merchandise from the HHCSRC Inc. will be consisting of powdered merchandise, extracts or concentrates and may be found in many forms, such as tablets, capsules, soft gels, gel caps, liquids and powders. 15. I have been fully informed that in future, only the user can discuss appointment via phone (no discussion about replacement/ return/refund) so that I can be given a convenient time. Also it won’t be possible to explain details to well wishers/friends. I have been told that whenever I call I should give reasonable amount of time for a return call as they have to be answered by authorized personnel only. I understand that during weekends and holidays or when there is problem with telecommunication there may be delay in getting an answer. 16. I agree that sales and services rights are reserved by the HHCSRC Inc., and its services can be denied if necessary. 17. Each merchandise package contains one-month / __months supply. After full payment the processing of Merchandise will start the same day and will be shipped as soon as possible. Prepared Merchandise have to be picked up within 7 days of the preparation. After 7 days, HHCSRC Inc. and/or its employee can’t be held responsible. 18. IMPORTANT NOTE FOR ADVISERS/WELL WISHERS: People come to us after exhausting all available options. THRE IS NO MAGIC BULLET. It will take time for these Merchandise to show effect. We highly appreciate each dollar spent from your hard earn money and sincerely hope are satisfied from the Merchandise you purchase. We cannot guarantee a supplement will help you; one may notice great benefit from a supplement, while other may not have desired benefit or may have a problem. I/we have read No Refund policy, CONTRACT OF DISCLOSURE, INFORMED CONSENT, DISCLAIMER, TERMS, CONDITIONS & REGISTRATION thoroughly and have understood the above before agreeing to this contract. I/we have read the contract documents WITHOUT ANY RUSH & understood all points. I/we understand that I am buying the Merchandise of my own free will after fully understanding & agreeing to the above terms. HHCSRC Inc. or its employees and volunteers have not done any fraud, misrepresentation, breach of trust or undue influence of any kind on me to extract money from me. I, my family & agents will not intentionally cause any damage to HHCSRC Inc. physically, financially or in reputation. AS I AGREE TO THIS DOCUMENT, I STATE THAT I AM NOT UNDER THE INFLUENCE OF ANY MEDICATIONS, ALCOHOL OR ANY OTHER DRUG OR UNDER PRESSURE OF ANY OTHER PERSON. I AM MENTALLY STABLE, NOT COGNITIVELY IMPAIRED, AM FULLY CAPABLE OF TAKING INDEPENDENT DECISIONS & MAKING PAYMENTS. I, my family and agents will NOT DISPUTE this later on for any reason. I am agreeing to the terms of this document willingly so that it can be used wherever required legally. I have read this contract completely, understood, acknowledge the contents and agree with this contract.

As soon as the Order is placed the mixing of supplements will be done and once they are mixed for buyer they cannot be either separated or used to any body else.
